Insulation to prevent moisture issues

I've got a project where I need to insulate two exterior walls as effectively as possible, but I also want to avoid running into moisture problems once the cold weather sets in. Adding more insulation is straightforward, but I know that if the wall assembly isn't designed right, condensation can build up and cause damage over time.
What's the best way to get solid insulation performance in such a way to keep the walls dry and healthy in winter?

A hybrid approach works best: use cavity insulation like mineral wool for R-value and fire resistance, then add continuous exterior rigid foam to keep the wall warm and reduce condensation risk.
